The Four Oaks PHASE Supervised Community Treatment is a delinquency specific program. PHASE programming incorporates state-of-the-art evidence-based interventions for juvenile offenders. Participating youth are able to stay connected to their community, schools, and families, and graduate from PHASE with reduced risk to reoffend.
This position is responsible to assesses clients interactions in the milieu; formulates interventions based on the identified treatment plan; evaluates the progress of treatment goals; provides direct care services on assigned unit(s) completing documentation as assigned; supports agency mission as well as program mission & values, practicing safe & responsible work habits.
Position Quailifications: Minimum High School Diploma/GED and one year experience. Preferred BA degree in a qualified field, or AA degree in a qualified field.
